You have two options with this form. You can either submit it electronically (for credit card orders only), or print the form and mail it in.
Once you have printed and completed the form, submit it to: Degree Services, Michigan Technological University, 1400 Townsend Drive, Houghton, MI 49931-1295 along with a check or money order made payable to MTU to cover processing fees (see details below). Please allow approximately two weeks for processing.
